Welcome to the submission form for your optional ORS Team Profile! To create your team’s profile, which will be accessible through the ORS website, fill in the form below. Content submissions for Quarter 2 2026 are due on March 25, 2026.

Submission Details:

• Each team should submit only once per quarter. Edits will no longer be permitted after the deadline indicated above. We highly recommend including content that will not require more frequent updates than quarterly.
• First-time submissions: Include all relevant content and details. Resubmissions: Make any necessary edits during each submission window using the link provided in your confirmation email.
• When applicable, all resources must be submitted and approved by ORS/CDC Foundation Clearance before adding them to your team’s profile.
• Each submission will undergo review and approval by the ORS Communications Team before it is reflected on your team’s profile.

Tips: Be sure to save your confirmation email, which contains your team’s content and unique submission link. For further guidance, consult the ORS Communications Toolkit and the Clearance Quick Reference Guide.

Your team has the option to upload your team’s resource as a file attachment or copy and paste a link in the URL section below. Be sure to add resources in the order you want them to appear and enter a title that best describes your item. Click “Add Another Resource” or the “plus icon” in the URL section to include additional files.

Select your state or jurisdiction from the dropdown menu; you may choose more than one if needed. Once selected, your state's name will appear in blue. After you submit the form and your profile is approved, any success stories your team has in the ORS News and Stories section of the ORS website will automatically appear on your team profile. If you would like to submit a new success story for consideration, please email orscommunications@cdcfoundation.org.

*After submitting your form, you’ll receive an automated confirmation email containing a copy of all your submitted information. At the bottom of that email, you’ll find a unique link you can use to edit your submission at any time during the open submission window. If you lose this link or need assistance, please contact orscommunications@cdcfoundation.org and we will resend you a notification with your unique link.
